Melasma and Chloasma

Melasma is blotchy irregular and has undefined borders occurring in a symmetrical fashion only on the face. Common areas include forehead, upper lip and upper cheeks. More common in olive to darker skin types. Predominantly occurs in females of childbearing age. Becomes daker in summer and lighter in winter.

Mainly hormonal, may appear during pregnancy known as Chloasma the 'mask of pregnancy.' Pigmentation is also exacerbated by contraceptive pills, HRT, medications, and sun-exposure.

Post-inflammatory Hyperpigmentation

Post inflammatory hyperpigmentation can range from mild to moderate erythema with patches or dark brown spots with an irregular outline. Can occur after trauma to the skin from sun exposure, cosmetic treatments from laser, and deep peels, acne, surgical incisions, cosmetics, topical prescription creams and photosensitising medications. 

 

Olive to darker skin types is more prone to developing Post inflammatory hyperpigmentation.  After the trauma it may take weeks or several years for the skin to return back to its usual colour.

Age Spots (Liver Spots)

Age spots are also called liver spots, sunspots, and solar lentigines, they are flat pigmented lesions, they vary in size and colour, ranging from light to dark brown and can cluster together. They are harmless and appear on sun exposed areas the face, back of hands, shoulders and arms.

The lesions start appearing in the third decade of adult life. Main cause is frequent and chronic sun exposure to UV radiation with little to no sun protection. The skin produces more pigment to protect itself from excess UV radiation and the pigment cells become overactive, therefore producing lesions.

Poikiloderma of Civette

Poikiloderma Of Civette is a combination of dilated fine blood vessels (telangiectasia) and hyperpigmentation. Usually appears as mottled, reddish-purple pigmentation on both sides of the neck and in a V-shape on the chest, is bumpy with fine lines.

 

Fairer skin types are prone to poikiloderma, menopausal women, genetics, the use of perfumes and chronic sun exposure.
